User Agreement

Registered users agree to the following conditions:


  1. have registration information reviewed by ChE Monitoring tools staff and shared with staff at the California Department of Pesticide Regulation and Office of Environmental Health Hazard Assessment (fulfills medical supervisor registration requirement included in AB 2892 - effective 1/1/2017)
  2. Comply with program policies on access to patient data & medical confidentiality.
  3. Respect permitted communications to employers.
  4. Compliance with open source software rules.
  5. Notification of individual patients in case of a breach of data.

Validation of license information with California Medical Board

Provider information submitted will be verified by review of public records with California Medical Board and related secondary data sources.

Access to Patient Data

ChE Monitoring Tools shall permit access to Patient Data maintained by only to approved participants and other parties authorized by the ChE Monitoring Tools, including California Public Agencies.

Compliance with medical confidentiality:

Participants agree to comply with provisions of the Health Information and Portability Act that apply to cholinesterase monitoring.

Privacy training to be completed annually by participating staff.

Locally stored information related to participating patients shall be stored in physically secure location, be password protected and protected by encryption.

Permitted communications to employers include:

Results of tests authorized under California regulations.

Activity restrictions required under monitoring protocol:

Return to unrestricted work activity.

Dates of follow-up testing appointments.
